Thank you for scheduling an appointment, a tentative appointment was created!
The Registered Dietitian will reach out via email to confirm your appointment within 48-hours. In order to receive your confirmation email, make sure to check your email for an "opt-in" message from Kalix, our HIPAA compliant electronic medical record system. If you use gmail, sometimes this email gets sent to the promotions folder, so make sure to keep an eye out for this email.

Once you have opted in to receive emails from me, you will immediately receive a confirmation email with your scheduled appointment time, a link to your appointment (if you've scheduled a virtual appointment), and a link to 2 forms (Hleap Nutrition Practice Policies and Intake Form) to fill out prior to your scheduled appointment.

If you have Keystone Health plan East or an HMO insurance plan, please make sure to obtain a referral from your doctor in order to ensure this service will be covered by your insurance. Here is a link to the form: REFERRAL FORM
